Medical Corps' Surgical Kit and Training Manual
Olsen Hegar Needle Holder TC. The needle holder is what drives the suture needle through the edges of the wound. This particular needle holder has Tungsten Carbide jaws and gold handles for easy identification.
The needle holder is one of the most used instruments in suturing. An "Olsen Hegar" has built in scissors that allows the surgeon to cut the suture after each "stitch" without laying the needle holder down and grabbing a pair of scissors.
Metzenbaum or Mayo Scissors. "Metzenbaums" and "Mayo" scissors are the second most used instrument. Scissors are used for trimming suture, cutting away dead tissue (debridement) and for minor surgery like resizing a wound or undermining. The scissors in the kit are curved blunt/blunt. |
Straight Kelly or Straight Rankin Hemostat. Straight hemostats are used for ligature (tying off), opening a wound and for extracting foreign bodies.
Curved Kelly. The curved Kelly is the most versatile for the two hemostats in the instrument set. They are used for ligature, enlarging a wound and checking a wound. |
Probe with Eye. It looks like a large sewing needle, but the end is blunted. The blunt end is used for gentling probing for foreign bodies like projectiles. For a puncture wound the blunt end is used to help guide gauze into the hole. If the puncture is thru-and-thru, the "eye" is used to guide the ribbon gauze through the puncture.
Tissue Forceps. While needle holders and hemostats are forceps too, the "tissue forceps" are large tweezers with small teeth for maneuvering tissue and helping work with the wound and to guide the suture needle.
#3 Scalpel Handle (Scalpel). A number 3 knife handle can be fitted with either a #10, #11, or #15 scalpel blade which comes with the kit.
The scalpel is how we make incisions. The #10 and #15 are used for surgical trimming. The #11 is used for lancing.
Disposable Scalpels. The kit also comes with a disposable scalpel for any emergency use.
Suture. We've included several packets of practice suture with the kit. The needles are large for easy learning and the suture material is silk which is easiest to handle.
All suture material is measured against silk for ease of handling. "Almost as good as silk." is a common comment. In reality, there is no such thing as "practice suture". In an emergency the 3-0 silk with reverse cutting needle can be used for suturing skin.
Suture and Surgical Hemostasis by Rebecca Pieknik.
For our last class we chose the book by Rebecca Pieknik for our main suturing reference. We include it with our the surgical instruments kit. Medical Corps still recommends that you acquire copies of Wounds and Lacerations by Trott as well as the Nursing Drug Handbook, Taber's Medical Dictionary, Where there is No Doctor, Where There is No Dentist and the Merck Manual.
